Our Dental Membership Plan
At Walnut Creek Family Dentistry, we offer a dental membership plan designed to provide affordability and access to quality dental care for our patients. With your comprehensive membership plan, there are:
- No yearly minimums
- No deductibles
- No claim forms
- No waiting periods
- No pre-authorizations needed
- No pre-existing exclusions

Program Guidelines
- The cost of the Walnut Creek Family Dental Membership Plan is subject to change
- Membership plan costs are payable in full at the time of registration
- The cost of the plan is NON-REFUNDABLE. No refunds will be issued if the patient elects not to utilize the plan
- The plan's effective date and record usage will be maintained by Walnut Creek Family Dentistry
- The patient's portion is due on the day of service
- CareCredit cannot be utilized as a payment method for this membership plan
- These plans and services are available to existing patients ONLY
Cost of Our Dental Plans
Adult Plan
Our adult plan is for ages 14 and up, and it costs $375 per year. Services are covered at 100% and include:
- Cleanings and exams (two per plan year)
- Bitewing X-rays (yearly/as needed)
- Pano X-rays (one every five years)
- Fluoride (one time per plan year)
- Limited oral exam (one per plan year)
- Intraoral X-rays (as needed)
Periodontal Plan
Our periodontal plan is for all patients following scaling and root planing, and it costs $525 per year. Services are covered at 100% and include:
- Periodontal maintenance (three per plan year)
- Exams (two per plan year)
- Bitewing X-rays (yearly/as needed)
- Pano X-rays (one every five years)
- Fluoride (one time per plan year)
- Limited oral exams (one per plan year)
- Intraoral X-rays (as needed)
Child Plan
Our child plan is for ages 0-13, and it costs $300 per year. Services are covered at 100% and include:
- Cleanings and exams (two per plan year)
- Bitewing X-rays (yearly/as needed)
- Pano X-rays (one every five years)
- Fluoride (one time per plan year)
- Limited oral exams (one per plan year)
- Intraoral X-rays (as needed)
All Other Procedures
All procedures listed below are 20% covered through our dental membership plans:
- Fillings
- Crowns/bridges/implant crowns*
- Periodontics*
- Dentures/partials
- Extractions (in-office)*
- Sealants
*A referral may be necessary for more complex procedures.
Exclusions and Limitations
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunctions with dental insurance
- For treatment that, in the sole opinion of the treating dentist, lies outside the realm of his capabilities
- Referrals to specialists (periodontists, oral surgeons, or endodontists)
- For the cost of dental care covered under automobile, homeowners, or medical insurance
It's important to highlight that all payment is due on the date of service. The membership plan is payable in full at registration.
